Our summer office hours are 8:00 a.m. – 2:00 p.m., Monday through Thursday. We are closed on Fridays through July.
Please bring the following items:
- Baptismal Certificate (if Catholic)
- Birth Certificate or Copy of Birth Certificate
- Current immunization records
